VIAL of Life
Dear Citizens of Nelson County,
On behalf of Nelson County EMS, we would like to encourage you and your family to become a participant in the Vial of Life (aka Vial of L.I.F.E. (Lifesaving Information for Emergencies)) Project. The Vial of Life is a FREE and simple tool that allows individuals to have their complete medical information ready in their home for emergency personnel to reference during an emergency.
Accidents are not planned, and they can happen to anyone. By planning ahead of time, our EMS mission to better serve the Citizens of Nelson County grows stronger.
When a 9-1-1 call is dispatched, EMS is only given the information the caller is able to provide. Keep in mind that in many situations the caller may not be able to provide critical information about you, the patient. Please read the following information to find out how you can better protect yourself by participating in the Vial of Life Project.
The Vial of Life is a single page, detailed form containing your medical information which is placed inside of your kitchen refrigerator in a specially marked container. Emergency responders can quickly identify a Vial of Life participant once you have completed the form below and placed the provided decal on the outside of your refrigerator.
QUESTION: Should I have a Vial of Life even if I have little or no medical conditions? ANSWER: Absolutely! Everyone in your home is encouraged to have the form completed and kept updated as needed
Nelson County EMS personnel will gladly answer any questions about the form and assist you in completing it if you are unable, just call (502) 348-4929.
